Generic Zyvox ( Linezolid )
Zyvox (linezolid) is an oxazolidinone antibiotic indicated for the treatment of serious infections caused by susceptible Gram‑positive bacteria, including nosocomial and community‑acquired pneumonia, complicated and uncomplicated skin and skin‑structure infections, and infections due to vancomycin‑resistant Enterococcus faecium (VRE). It works by selectively binding to the 23S ribosomal RNA of the 50S bacterial ribosomal subunit, preventing the formation of the functional 70S initiation complex and thereby inhibiting bacterial protein synthesis. Linezolid is bacteriostatic against enterococci and staphylococci and bactericidal against most strains of streptococci. It is reserved for infections where alternative agents are not appropriate due to resistance or intolerance.
Usual adult dose: The recommended dose is 600 mg every 12 hours, administered orally or by intravenous infusion over 30 to 120 minutes. The duration of treatment is typically 10 to 14 days for complicated skin infections and pneumonia, and 14 to 28 days for VRE infections. Uncomplicated skin infections may be treated with 400 mg every 12 hours; however, the 600 mg tablet is the standard strength for most serious infections. No dose adjustment is required when switching from intravenous to oral therapy. Linezolid may be taken with or without food. In patients with severe renal or hepatic impairment, caution is warranted although formal dose adjustment is not required; accumulation of metabolites may occur.
Dosage form: Film‑coated tablets: 600 mg (white to off‑white, oval, debossed). Also available as a 2 mg/mL intravenous injection solution and as a powder for oral suspension (100 mg/5 mL when reconstituted) for paediatric patients or those unable to swallow tablets.
Onset of action: Peak plasma concentrations are achieved within 1 to 2 hours of oral dosing. Steady‑state levels are reached by the second day of twice‑daily administration. Clinical response in susceptible infections may be evident within the first 48 to 72 hours.
Duration of action: The elimination half‑life is approximately 5 hours, supporting a twice‑daily dosing interval. Therapeutic levels are maintained throughout a 12‑hour period.
Alcohol recommendation: Alcohol consumption must be strictly avoided during treatment with linezolid and for 48 hours after the last dose. Linezolid is a reversible, nonselective inhibitor of monoamine oxidase (MAO), and ingestion of alcoholic beverages with high tyramine content—especially tap beers, red wine, and sherry—can precipitate a hypertensive crisis. Patients should also avoid other tyramine‑rich foods as directed by their healthcare professional.
Most common side effects: Diarrhea, nausea, vomiting, headache, and taste disturbance. Myelosuppression (reversible thrombocytopenia, anemia, and leukopenia) is a significant adverse effect and is more likely with treatment courses exceeding 2 weeks; complete blood counts should be monitored weekly. Lactic acidosis, peripheral and optic neuropathy have been reported with prolonged use; patients should report any vision changes, numbness, or tingling. Serotonin syndrome may occur when linezolid is co‑administered with selective serotonin reuptake inhibitors (SSRIs), serotonin‑norepinephrine reuptake inhibitors (SNRIs), or other serotonergic agents; co‑administration is generally contraindicated unless strict monitoring is in place. Zyvox must be used with caution in patients with uncontrolled hypertension, pheochromocytoma, or thyrotoxicosis due to the risk of MAO‑related blood pressure elevation.
