Generic Vermox ( Mebendazole )
Vermox (mebendazole) is a broad‑spectrum anthelmintic indicated for the treatment of single or mixed gastrointestinal infestations caused by Enterobius vermicularis (pinworm), Trichuris trichiura (whipworm), Ascaris lumbricoides (roundworm), Ancylostoma duodenale, and Necator americanus (hookworm). Mebendazole works by binding to parasite tubulin and inhibiting microtubule assembly, which leads to impaired glucose uptake, depletion of glycogen stores, and eventual death of the worm. It acts locally in the intestinal lumen, and systemic absorption is minimal. Vermox is available without a prescription in many parts of Canada for the treatment of pinworm, but other indications may require medical assessment.
Usual adult dose: For pinworm (enterobiasis), the recommended dose is 100 mg taken as a single dose. Because reinfection is common, a second 100 mg dose should be taken 2 to 3 weeks later, and all household contacts should be treated simultaneously. For whipworm, roundworm, and hookworm infections, the dose is 100 mg taken in the morning and evening for 3 consecutive days. If the infection is not cured 3 weeks after treatment, a second course may be given. The tablets may be chewed, swallowed whole, or crushed and mixed with food; they can be taken with or without food. No dose adjustment is required in renal or hepatic impairment, but caution is advised in severe hepatic dysfunction due to limited data.
Dosage form: Chewable tablets: 100 mg (white to off‑white, round, with a score line). Available in blister packs or bottles of 6 tablets.
Onset of action: Mebendazole acts directly on intestinal parasites within hours of ingestion. Clinical clearance of worms from the gastrointestinal tract typically occurs within 1 to 3 days of treatment, depending on the parasite load and the indication. Symptomatic relief of pruritus in pinworm infection may begin within 24 to 48 hours.
Duration of action: The anthelmintic effect is localized and short‑lived; the elimination half‑life of mebendazole is approximately 2.5 to 5.5 hours. Because the drug is poorly absorbed and acts within the gut lumen, repeat dosing or a second course is often required for complete eradication and prevention of reinfection.
Alcohol recommendation: There is no known direct interaction between mebendazole and alcohol. However, alcohol may irritate the gastrointestinal tract and worsen abdominal discomfort, nausea, or diarrhea that can occur with intestinal parasitic infections. Patients may limit alcohol until treatment is completed and symptoms resolve.
Most common side effects: Abdominal pain, diarrhea, flatulence, nausea, and vomiting. These effects are generally mild and transient. Headache and dizziness may also occur. Rare but serious adverse events include agranulocytosis, hepatitis, and seizures, particularly with high or prolonged dosing. Vermox is contraindicated in patients with known hypersensitivity to mebendazole or any component of the formulation. It should not be used during pregnancy, especially in the first trimester, unless the potential benefit clearly outweighs the risk. Breastfeeding women should consult a healthcare professional before use. Because pinworm spreads easily within households, strict hygiene measures, including handwashing, washing of bed linens, and treatment of all close contacts, are essential to prevent recurrence.
