Generic Pyridium ( Phenazopyridine )
Pyridium (phenazopyridine hydrochloride) is a urinary tract analgesic indicated for the symptomatic relief of dysuria, burning, urgency, frequency, and pain associated with lower urinary tract irritation caused by infection, trauma, surgery, or instrumentation. It exerts a local anesthetic or analgesic effect directly on the urinary tract mucosa, but it has no antibacterial activity and is not a treatment for urinary tract infection; it is used only as an adjunct to appropriate antimicrobial therapy when infection is present. Phenazopyridine is available only by prescription in Canada in the 200 mg strength, while lower strengths may be available without a prescription in some countries.
Usual adult dose: The recommended dose is 200 mg taken orally three times daily after meals. The tablets should be swallowed whole with water. Treatment should not exceed 2 days when used in combination with an antibacterial for urinary tract infection, and longer use requires medical reassessment. If a dose is missed, it should be taken as soon as remembered, but two doses should not be taken at the same time. Patients should be advised that phenazopyridine causes a harmless reddish-orange discolouration of urine and may stain clothing and contact lenses. No dose adjustment is required in mild to moderate renal impairment; however, it is contraindicated in severe renal impairment (creatinine clearance less than 30 mL/min) and in patients with hepatic impairment.
Dosage form: Film-coated tablets: 200 mg (dark red, round, biconvex, often debossed with “PYR” or “200”). Available in bottles of 30 or 100 tablets.
Onset of action: Symptomatic relief of urinary pain and burning typically begins within 30 to 60 minutes after oral administration, with peak effects at 1 to 2 hours. The onset is rapid and patients usually experience improvement in dysuria shortly after the first dose.
Duration of action: The analgesic effect lasts for approximately 4 to 6 hours, supporting three-times-daily dosing. The elimination half-life of phenazopyridine is approximately 2 to 3 hours in patients with normal renal function but is prolonged in renal impairment. The therapeutic effect persists only as long as the drug is taken regularly.
Alcohol recommendation: Alcohol should be avoided during phenazopyridine therapy. Alcohol may exacerbate gastrointestinal irritation and increase the risk of stomach upset, dizziness, and drowsiness. It may also worsen underlying urinary tract inflammation and dehydration. Patients should limit alcohol intake while using this medication.
Most common side effects: Headache, dizziness, and gastrointestinal upset (nausea, vomiting, abdominal pain). The urine and sometimes skin or sclera may become orange or red in colour; this is harmless but can be alarming. Rare but serious adverse events include methemoglobinemia (bluish skin, shortness of breath, confusion), hemolytic anemia, and renal toxicity, particularly with overdose or in patients with glucose-6-phosphate dehydrogenase (G6PD) deficiency. Phenazopyridine is contraindicated in patients with known hypersensitivity, severe renal impairment, or G6PD deficiency. It should not be used for more than 2 days when combined with an antibacterial. If symptoms persist beyond 2 days, the patient must be re-evaluated by a healthcare professional. Pyridium is available only by prescription in Canada and should be used under medical supervision.
