Podowart ( Podophyllum resin )
Podowart (podophyllum resin) is a plant-derived cytotoxic agent indicated for the topical treatment of external genital and perianal warts (condyloma acuminata). The active principle, podophyllotoxin, binds to tubulin and arrests cell division in metaphase, leading to necrosis of the wart tissue. It also inhibits nucleoside transport, further impairing the proliferation of virus‑infected keratinocytes. In Canada, podophyllum resin preparations are prescription medicines and should be applied under the guidance of a healthcare professional, with initial application often performed in the clinic to assess the reaction.
Usual adult dose: The solution is applied sparingly to each wart using a cotton-tipped applicator or the provided brush, taking extreme care to avoid contact with healthy skin. For the first treatment, the applied area is left uncovered and allowed to air‑dry; after 1 to 4 hours, the resin must be thoroughly washed off with soap and water. For subsequent weekly applications, the contact time may be reduced to 1 to 2 hours, depending on tolerance. The total volume applied per session should not exceed 0.5 mL, and the total treated area should be no more than 10 cm². Treatment is repeated once weekly for up to 6 weeks or until the warts resolve. If excessive local irritation occurs, the interval between applications should be extended. Podophyllum resin should not be applied to bleeding, friable, or recently biopsied warts, and it must never be used on mucous membranes, internal genital surfaces, or large areas of skin.
Dosage form: Topical solution: podophyllum resin 10% to 25% w/v in compound tincture of benzoin. Available in a 10 mL bottle with an applicator.
Onset of action: Necrosis and blanching of the wart tissue typically begin within 24 to 48 hours of application, with visible shrinkage and sloughing occurring over several days. Repeated weekly treatments are required for complete clearance.
Duration of action: Each application provides local cytotoxic action that persists for hours until the resin is washed off. The full therapeutic effect is assessed after a course of up to 6 weekly treatments; no residual pharmacological activity remains between treatments.
Alcohol recommendation: There is no known systemic interaction between topical podophyllum resin and alcoholic beverages. However, patients must avoid applying any alcohol‑based products, astringents, or other irritants to the treatment area before or after application, as they can intensify pain and local tissue damage.
Most common side effects: Expected local reactions include burning, stinging, redness, tenderness, and superficial erosion at the application site. These are usually mild to moderate when the product is used correctly. Over‑application, prolonged contact time, or application to large areas can lead to severe local ulceration, secondary infection, and systemic toxicity (manifesting as nausea, vomiting, peripheral neuropathy, bone marrow depression, and even fatal encephalopathy). Systemic absorption is increased if the solution is applied to abraded or inflamed skin. Podophyllum resin is absolutely contraindicated during pregnancy (FDA Category X) due to the risk of fetal death and teratogenicity; a negative pregnancy test and effective contraception are required before use in females of childbearing potential. The product is also contraindicated in breastfeeding women and in patients with diabetes mellitus, poor circulation, or known hypersensitivity. Patients must be instructed to never use more than the directed amount, to wash off the resin at the specified time, and to seek immediate medical attention if signs of systemic toxicity or severe local reaction occur.
