Generic Panadol ( Paracetamol )
Panadol (paracetamol, also known as acetaminophen in Canada) is an analgesic and antipyretic indicated for the temporary relief of mild to moderate pain and the reduction of fever. It works by inhibiting prostaglandin synthesis in the central nervous system and by acting on the hypothalamic heat-regulating centre, providing effective relief from headache, toothache, muscle aches, back pain, menstrual cramps, and fever. Unlike nonsteroidal anti-inflammatory drugs (NSAIDs), paracetamol has minimal anti-inflammatory activity and does not irritate the stomach lining. Panadol is available without a prescription in Canada.
Usual adult dose: The recommended dose is 500 mg to 1000 mg taken orally every 4 to 6 hours as needed, up to a maximum of 4000 mg per day. The 500 mg tablet is the standard over-the-counter strength. No more than 8 tablets (4000 mg) should be taken in any 24-hour period unless directed by a healthcare professional. The tablet should be swallowed whole with water and may be taken with or without food; taking it with food may reduce stomach upset in sensitive individuals. Treatment should be limited to short-term use. If fever persists for more than 3 days or pain for more than 5 days, the patient should consult a healthcare professional. Patients with liver disease, those who consume three or more alcoholic beverages per day, or those taking other medications containing acetaminophen should not exceed a lower total daily dose unless directed by a physician.
Dosage form: Film-coated tablets: 500 mg (white, capsule-shaped or round). Available in blister packs or bottles of 12, 24, 50, or 100 tablets. Panadol is also available as caplets, liquid gels, chewable tablets, and oral suspension for children under other brand names.
Onset of action: Analgesic and antipyretic effects typically begin within 30 to 60 minutes after oral administration, with peak plasma concentrations reached at approximately 1 to 2 hours.
Duration of action: The clinical effect lasts approximately 4 to 6 hours, supporting dosing every 4 to 6 hours as needed. The elimination half-life of paracetamol is about 2 to 3 hours in healthy adults.
Alcohol recommendation: Alcohol should be avoided or strictly limited while taking Panadol. Chronic or excessive alcohol consumption increases the risk of severe liver injury from paracetamol, even at doses that are normally safe. Patients who consume three or more alcoholic drinks per day should consult a healthcare professional before using paracetamol and should use the lowest effective dose for the shortest possible duration.
Most common side effects: Paracetamol is generally well tolerated at recommended doses. Nausea, vomiting, constipation, and rash occur rarely. The most serious adverse effect is hepatotoxicity, which is usually associated with overdose (single ingestion of more than 4000 mg in adults) or with prolonged use at high doses. Early signs of liver damage may include loss of appetite, nausea, vomiting, abdominal pain, and jaundice; immediate medical attention is required. Patients should be cautioned not to take Panadol together with other products containing acetaminophen, including many over-the-counter cold and flu preparations. In Canada, paracetamol is widely used and is available without a prescription, but all labelled dosing instructions should be followed carefully.
