Generic Orapred ( Prednisolone )
Orapred (prednisolone sodium phosphate) is a synthetic adrenocorticosteroid with potent anti-inflammatory and immunosuppressive properties. It is indicated for a wide range of conditions requiring corticosteroid therapy, including allergic disorders, dermatologic diseases, endocrine disorders, gastrointestinal diseases (such as ulcerative colitis), hematologic disorders, neoplastic diseases, nervous system disorders, ophthalmic diseases, renal diseases, respiratory diseases (including asthma), and rheumatologic disorders. Prednisolone works by binding to intracellular glucocorticoid receptors, modulating gene transcription to reduce inflammatory mediators such as prostaglandins and leukotrienes, suppress migration of inflammatory cells, and stabilize lysosomal membranes. Orapred is available as an oral solution and orally disintegrating tablets, providing flexible dosing for patients who have difficulty swallowing standard tablets.
Usual adult dose: The dosage is highly individualized based on the disease, its severity, and patient response. Initial doses generally range from 5 mg to 60 mg daily in divided doses, with higher doses for acute severe conditions. A common starting dose for many inflammatory conditions is 20 mg to 40 mg per day. The 2.5 mg and 5 mg strengths are used for fine dose titration and paediatric dosing; the 10 mg, 20 mg, and 40 mg strengths allow convenient higher-dose administration. Once a satisfactory response is achieved, the dose is gradually tapered to the lowest effective maintenance dose, often given as a single morning dose or on alternate days to minimize hypothalamic-pituitary-adrenal (HPA) axis suppression. Abrupt discontinuation after prolonged therapy may precipitate acute adrenal insufficiency; gradual withdrawal is essential. For acute asthma exacerbations, a short course of 40 mg to 60 mg daily for 5 to 10 days may be used. Doses for children are based on body weight or surface area.
Dosage form: Orally disintegrating tablets: 2.5 mg, 5 mg, 10 mg, 20 mg, and 40 mg (as prednisolone sodium phosphate). Also available as an oral solution (Orapred) containing 15 mg/5 mL prednisolone sodium phosphate, and as standard oral tablets of prednisolone under other brand names. The orally disintegrating tablet should be placed on the tongue, allowed to dissolve, and then swallowed with saliva; no water is needed. The solution should be measured with a calibrated oral syringe or dosing cup.
Onset of action: The anti-inflammatory and immunosuppressive effects begin within hours of oral administration, but the full clinical response for many chronic conditions may require several days of therapy. For acute asthma, improvement in forced expiratory volume may be seen within 6 to 12 hours.
Duration of action: Prednisolone has an intermediate duration of action, with a biological half-life of 18 to 36 hours, allowing for once-daily or twice-daily dosing. Its therapeutic effects persist beyond the plasma elimination half-life due to the genomic mechanisms of action.
Alcohol recommendation: While moderate alcohol consumption is not absolutely contraindicated, alcohol should be used with caution during prednisolone therapy. Corticosteroids increase the risk of gastrointestinal irritation and bleeding, and alcohol can exacerbate this risk. Patients should discuss their alcohol intake with their healthcare provider.
Most common side effects: Short-term use may cause fluid retention, increased appetite, weight gain, insomnia, mood changes (including euphoria or depression), headache, and elevated blood glucose. Long-term use, especially at high doses, can lead to more serious effects: osteoporosis, cataracts, glaucoma, Cushingoid features (moon face, buffalo hump, truncal obesity), thinning of the skin, easy bruising, muscle wasting, peptic ulcer disease, impaired wound healing, increased susceptibility to infections, and suppression of the HPA axis. Patients should be monitored for signs of hyperglycemia, particularly those with diabetes mellitus. Live or live-attenuated vaccines should not be administered during high-dose corticosteroid therapy. To reduce gastrointestinal upset, prednisolone should be taken with food or milk. Abrupt withdrawal after extended use may cause fever, myalgia, arthralgia, and potentially life-threatening adrenal crisis.
