Generic Mobic ( Meloxicam )
Mobic (meloxicam) is a nonsteroidal anti-inflammatory drug (NSAID) indicated for the relief of signs and symptoms of rheumatoid arthritis and osteoarthritis. Meloxicam works by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, with preferential inhibition of COX-2 at lower therapeutic doses, reducing the production of prostaglandins responsible for pain, inflammation, and swelling. Mobic is available in tablet and oral suspension forms and is intended for long-term management of chronic arthritic conditions, not for acute pain relief. It offers the convenience of once-daily dosing due to its long half-life.
Usual adult dose: For osteoarthritis and rheumatoid arthritis, the recommended starting and maintenance dose is 7.5 mg taken orally once daily. If the response is inadequate after 4 to 6 weeks, the dose may be increased to 15 mg once daily. The maximum recommended daily dose is 15 mg. Mobic should be taken with food or milk to reduce gastrointestinal upset, and the tablet should be swallowed whole with water. The 7.5 mg and 15 mg strengths are available as tablets. Patients should be re-evaluated periodically, and therapy should be continued at the lowest effective dose for the shortest duration necessary. No dose adjustment is required in mild to moderate renal or hepatic impairment, but caution is advised in severe impairment; meloxicam is not recommended in severe renal impairment or in patients with creatinine clearance below 30 mL/min.
Dosage form: Oral tablets: 7.5 mg (yellow, round, biconvex, debossed with "7.5") and 15 mg (yellow, oval, biconvex, debossed with "15"). Mobic is also available as an oral suspension (7.5 mg/5 mL) for patients who cannot swallow tablets. The 7.5 mg and 15 mg strengths are the standard once-daily formulations in Canada.
Onset of action: The analgesic and anti-inflammatory effect begins within 30 to 60 minutes of oral administration, with peak plasma concentrations reached at approximately 4 to 5 hours. For chronic arthritic conditions, full therapeutic benefit may take 2 to 4 weeks of continuous daily therapy to develop. Meloxicam is not intended for rapid relief of acute pain.
Duration of action: The elimination half-life of meloxicam is approximately 20 hours, supporting once-daily dosing. The clinical effect persists for up to 24 hours after a single dose, maintaining symptom control throughout the day. Steady-state plasma levels are achieved after 3 to 5 days of once-daily administration.
Alcohol recommendation: Alcohol consumption should be limited or avoided during treatment with Mobic. Consuming three or more alcoholic beverages per day while taking an NSAID increases the risk of serious gastrointestinal bleeding, ulceration, and perforation, which can be fatal. Alcohol may also increase the risk of liver injury, dizziness, and drowsiness. Patients should be counselled about these risks, especially if they have a history of peptic ulcer disease or liver disease.
Most common side effects: Dyspepsia, nausea, abdominal pain, diarrhea, headache, dizziness, and peripheral edema. Gastrointestinal side effects are common; taking the medication with food may reduce them. Meloxicam, like all NSAIDs, is associated with an increased risk of serious cardiovascular thrombotic events (myocardial infarction, stroke) and serious gastrointestinal adverse events (bleeding, ulceration, perforation), particularly with long-term use or in patients with pre-existing cardiovascular or gastrointestinal risk factors. It is contraindicated in patients with known hypersensitivity to meloxicam or other NSAIDs, in the setting of coronary artery bypass graft surgery, and in patients with active peptic ulcer disease or gastrointestinal bleeding. Caution is required in patients with renal impairment, heart failure, hypertension, or those taking anticoagulants, diuretics, or ACE inhibitors. Patients should seek immediate medical attention for signs of gastrointestinal bleeding (black stools, vomiting blood), chest pain, shortness of breath, or sudden weakness. Mobic is available only by prescription in Canada and must be prescribed and monitored by a healthcare professional.
