Megalis ( Tadalafil )
Megalis (tadalafil) is a phosphodiesterase type 5 (PDE5) inhibitor that contains the same active ingredient as Cialis. While tadalafil is approved in Canada for erectile dysfunction and benign prostatic hyperplasia, Megalis itself is a foreign‑manufactured product that is not authorized for sale by Health Canada. Its safety, quality, and efficacy have not been evaluated by Canadian health authorities, and it is frequently encountered through unregulated online sources. Tadalafil works by enhancing the effects of nitric oxide released during sexual stimulation, increasing cGMP levels in the corpus cavernosum, leading to smooth muscle relaxation, vasodilation, and increased penile blood flow. It is effective only in the presence of sexual arousal and is distinguished from other PDE5 inhibitors by its longer duration of action.
Usual adult dose: In a regulated prescription setting, the recommended starting dose of tadalafil for erectile dysfunction is 10 mg taken as needed at least 30 minutes before sexual activity. Based on response and tolerability, the dose may be increased to a maximum of 20 mg or decreased to 5 mg. The maximum dosing frequency is once daily. For once‑daily use (not as needed), the dose is 2.5 mg or 5 mg taken at approximately the same time each day, without regard to timing of sexual activity. The 20 mg dose is typically reserved for as‑needed use. Megalis 20 mg should not be taken more than once in 24 hours. A high‑fat meal does not significantly affect absorption. Concomitant use with nitrates is absolutely contraindicated.
Dosage form: Film‑coated tablets: 20 mg (as tadalafil). Megalis tablets are typically yellow, almond‑shaped, and similar in appearance to authentic Cialis 20 mg products; however, the identity of the manufacturer and the tablet’s content cannot be verified through Canadian regulatory channels.
Onset of action: Onset of effect is typically within 30 to 60 minutes after oral administration, with tadalafil concentration peaking at a median time of 2 hours. Food does not affect the extent of absorption, so onset is relatively consistent regardless of meals.
Duration of action: Tadalafil has a uniquely long duration of effect, lasting up to 36 hours in some men. The elimination half‑life is approximately 17.5 hours. Priapism, an erection lasting longer than 4 hours, is a medical emergency.
Alcohol recommendation: Alcohol should be limited or avoided while taking tadalafil. Alcohol can potentiate the vasodilatory effects, increasing the risk of dizziness, orthostatic hypotension, headache, and syncope. Excessive drinking may also impair the ability to achieve an erection.
Most common side effects: Headache, dyspepsia, back pain, myalgia, nasal congestion, facial flushing, and dizziness. Back pain and muscle aches are more characteristic of tadalafil than other PDE5 inhibitors and typically resolve within 48 hours. Visual disturbances are uncommon. Tadalafil is absolutely contraindicated in patients taking any form of nitrates (e.g., nitroglycerin, isosorbide mononitrate) because the combined vasodilatory effect can precipitate life‑threatening hypotension. It should not be used in patients with severe cardiovascular disease, recent stroke or myocardial infarction, severe hepatic impairment, or retinitis pigmentosa. Rare but serious events include non‑arteritic anterior ischemic optic neuropathy (sudden vision loss) and sudden hearing loss. Because Megalis is an unapproved product obtained outside Canada’s regulated pharmaceutical supply, there is an added risk of substandard manufacturing, contamination, incorrect dosing, and the presence of undisclosed or toxic substances. Only tadalafil products prescribed by a licensed Canadian healthcare professional and dispensed by a licensed pharmacy should be used.
