Generic Ibuprofen
Ibuprofen is a nonsteroidal anti-inflammatory drug (NSAID) indicated for the temporary relief of mild to moderate pain and the reduction of fever. It works by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, which reduces the production of prostaglandins responsible for pain, inflammation, and elevated body temperature. Ibuprofen provides effective relief from headaches, dental pain, menstrual cramps, muscle aches, and minor arthritis pain. It is available both over the counter (200 mg) and by prescription (400 mg, 600 mg, and 800 mg) for more severe or chronic conditions.
Usual adult dose: For minor aches and pains, the recommended over-the-counter dose is 200 mg to 400 mg orally every 4 to 6 hours as needed. The maximum over-the-counter daily dose is 1200 mg per day unless directed by a healthcare professional. Prescription strengths (400 mg, 600 mg, or 800 mg) may be used for conditions requiring higher doses, typically 600 mg to 800 mg three to four times daily, with a maximum prescription daily dose of 3200 mg under medical supervision. Ibuprofen should be taken with food or milk to minimize gastrointestinal upset. The 600 mg strength is not available over the counter and must be prescribed.
Dosage form: Available as over-the-counter tablets (200 mg), caplets (200 mg), liquid gels (200 mg), and chewable tablets for children (dosed by weight). Prescription-strength tablets of 400 mg, 600 mg, and 800 mg are also available. The 600 mg tablet is a common prescription strength for moderate pain and inflammation.
Onset of action: Analgesic and antipyretic effects typically begin within 30 to 60 minutes of oral administration, with peak plasma concentrations reached in 1 to 2 hours. For inflammatory conditions, several days of regular dosing may be needed for maximum anti-inflammatory benefit.
Duration of action: Approximately 4 to 6 hours for over-the-counter doses, supporting a four-times-daily or as-needed dosing regimen. Prescription doses may be taken every 6 to 8 hours depending on the strength. The elimination half-life is about 1.8 to 2 hours.
Alcohol recommendation: Alcohol consumption should be limited or avoided during treatment with ibuprofen. Consuming three or more alcoholic beverages per day while taking ibuprofen may increase the risk of gastrointestinal bleeding, ulceration, and perforation, which can be fatal. Patients should also be aware of increased risk of liver injury and drowsiness.
Most common side effects: Stomach upset, heartburn, nausea, dizziness, and mild headache. Taking the medication with food or milk can help reduce gastrointestinal side effects. Long-term or high-dose use can increase the risk of serious cardiovascular thrombotic events, myocardial infarction, and stroke, as well as serious gastrointestinal adverse events including bleeding and ulceration. Ibuprofen is contraindicated in patients with known hypersensitivity to NSAIDs, in the setting of coronary artery bypass graft surgery, and in those with active gastrointestinal bleeding. Patients should seek immediate medical attention for signs of bleeding (black stools, vomiting blood), chest pain, or sudden weakness.
