Generic Fluticasone Propionate
Fluticasone propionate is a synthetic corticosteroid with potent anti-inflammatory activity indicated for the maintenance prophylactic treatment of bronchial asthma in patients requiring long-term control. It works by binding to glucocorticoid receptors, inhibiting the release of inflammatory mediators such as histamine, leukotrienes, and cytokines, and suppressing the late and early phase asthmatic reactions. This reduces airway hyperresponsiveness, bronchospasm, and mucus production. Fluticasone propionate is not indicated for the relief of acute bronchospasm.
Usual adult dose: The dosage is individualized based on asthma severity. For dry powder inhalers, the recommended starting dose for patients not previously on inhaled corticosteroids is 100 mcg twice daily. For patients switching from other inhaled corticosteroids, the starting dose may range from 100 mcg to 250 mcg twice daily. The maximum recommended dose is 500 mcg twice daily. For severe asthma, a dose of up to 1000 mcg twice daily may be considered under specialist supervision. After stabilization, the dose should be titrated to the lowest effective dose that maintains asthma control. Patients should rinse their mouth with water and spit it out after each inhalation to reduce the risk of oropharyngeal candidiasis and dysphonia.
Dosage form: Available as dry powder for oral inhalation in pre-metered dose blister strips or devices delivering 50 mcg, 100 mcg, or 250 mcg per inhalation. Also available as a pressurized metered-dose aerosol inhaler and a nebulizer suspension.
Onset of action: Improvement in asthma symptoms and pulmonary function may occur within 24 hours of initiating therapy, but maximum therapeutic benefit is typically achieved after 1 to 2 weeks of continuous treatment. Patients should be made aware that this medication must be used regularly for full prophylactic effect.
Duration of action: The therapeutic effects persist for a number of hours, permitting twice-daily maintenance dosing. The elimination half-life is approximately 7.8 hours following intravenous administration, but the clinical duration is governed by local pulmonary receptor binding.
Alcohol recommendation: There is no known direct interaction between fluticasone propionate inhalers and alcohol. However, excessive alcohol consumption may exacerbate certain underlying conditions such as gastroesophageal reflux disease (GERD), which can mimic or worsen asthma symptoms.
Most common side effects: Oropharyngeal candidiasis (thrush), dysphonia (hoarseness), headache, and throat irritation. The incidence of local side effects can be minimized by rinsing the mouth with water and spitting it out after each use. Corticosteroid inhalers at high doses for prolonged periods may cause systemic effects including adrenal suppression, growth retardation in children, decreased bone mineral density, cataracts, and glaucoma. The dose should be maintained at the lowest effective level to minimize potential systemic side effects. Paradoxical bronchospasm may occur immediately after dosing and should be managed with a short-acting bronchodilator; fluticasone propionate should be discontinued if this occurs. Patients with active or quiescent pulmonary tuberculosis or untreated systemic fungal, bacterial, viral, or parasitic infections should use with caution. This medication should not be used as the sole therapy for status asthmaticus or other acute episodes of asthma.
