Generic Femara ( Letrozole )
Femara (letrozole) is a non‑steroidal aromatase inhibitor indicated for the adjuvant treatment of postmenopausal women with hormone receptor‑positive early breast cancer, for the extended adjuvant treatment of early breast cancer in postmenopausal women who have received prior standard adjuvant tamoxifen therapy, for first‑line treatment of postmenopausal women with hormone receptor‑positive or hormone receptor‑unknown locally advanced or metastatic breast cancer, and for advanced breast cancer in postmenopausal women with disease progression following antiestrogen therapy. Letrozole works by selectively inhibiting the aromatase enzyme, which converts androgens to estrogens in peripheral tissues. In postmenopausal women, this leads to profound suppression of circulating estrogen levels, thereby reducing estrogen‑driven tumour growth. Femara is not effective in premenopausal women and should not be used in this population.
Usual adult dose: The recommended dose is 2.5 mg taken orally once daily, with or without food. Treatment is continued until disease progression or unacceptable toxicity in the metastatic setting; in the adjuvant setting, the recommended duration is generally 5 years, or as determined by the treating oncologist. No dose adjustment is required for elderly patients or for those with mild to moderate renal or hepatic impairment. In patients with severe hepatic impairment or severe renal impairment, use should be undertaken with caution. If a dose is missed, the patient should take the next dose at the usual time; two doses must not be taken on the same day. The tablet should be swallowed whole with water.
Dosage form: Film‑coated tablets: 2.5 mg (yellow, round, slightly biconvex, film‑coated). Available in blister packs or bottles of 30 or 100 tablets.
Onset of action: Estrogen suppression begins within the first days of treatment; circulating estradiol levels are reduced by approximately 70% to 80% within 2 weeks of daily dosing. Steady‑state plasma concentrations of letrozole are reached within 2 to 6 weeks. Clinical benefit in breast cancer, such as tumour regression or delay in disease progression, is typically assessed after several weeks to months of continuous therapy.
Duration of action: The elimination half‑life of letrozole is approximately 2 to 4 days, supporting once‑daily dosing. Estrogen suppression is maintained throughout the dosing interval and persists with continuous daily therapy. After discontinuation, estradiol levels gradually return to baseline over several weeks.
Alcohol recommendation: No direct interaction between letrozole and alcohol has been clearly established. However, excessive alcohol consumption may increase the risk of hot flashes, contribute to bone loss, and impair overall treatment tolerance. Patients should limit alcohol intake and discuss any concerns with their oncologist.
Most common side effects: Hot flashes, asthenia (weakness), arthralgia (joint pain), arthritis, bone pain, headache, nausea, fatigue, and peripheral edema. Musculoskeletal symptoms are frequent and may require symptomatic management. Letrozole reduces circulating estrogen and therefore causes loss of bone mineral density; osteoporosis and increased fracture risk are significant long‑term concerns. Bone density should be assessed at baseline and periodically, and calcium and vitamin D supplementation as well as bisphosphonates may be considered. Other effects include hypercholesterolemia, mood changes, depression, hypertension, and vaginal dryness. In women with pre‑existing ischemic heart disease, letrozole has been associated with a slightly increased risk of cardiovascular events. Because Femara suppresses estrogen to near undetectable levels, it is not effective in premenopausal women; a reliable method of contraception is required in women of childbearing potential if used off‑label. Femara is available only by prescription and must be prescribed and monitored by a healthcare professional experienced in oncology.
