Generic Femalegra ( Sildenafil Citrate )
Femalegra (sildenafil citrate) is a phosphodiesterase type 5 (PDE5) inhibitor that contains the same active ingredient as Viagra. While sildenafil is primarily approved in Canada for the treatment of erectile dysfunction in men, products under the name Femalegra are often marketed for female sexual dysfunction; however, they are not approved by Health Canada for use in women. Sildenafil works by enhancing the effects of nitric oxide released during sexual stimulation, which increases cyclic guanosine monophosphate (cGMP) levels in the corpus cavernosum (in men) or genital tissues (in women), leading to smooth muscle relaxation, vasodilation, and increased blood flow. It is effective only in the presence of sexual arousal and does not increase libido. Femalegra 100 mg is a high‑dose formulation and is typically used on an as‑needed basis.
Usual adult dose: For erectile dysfunction, the recommended starting dose is 50 mg taken as needed, approximately 1 hour before sexual activity. Based on effectiveness and tolerability, the dose may be increased to a maximum of 100 mg or decreased to 25 mg. The maximum recommended dosing frequency is once daily. For female sexual dysfunction, no officially approved dose exists; if used off‑label, the same dosing principles apply under strict medical supervision. A high‑fat meal may delay the onset of action. Femalegra 100 mg must not be taken more than once in 24 hours.
Dosage form: Oral tablets: 100 mg (as sildenafil citrate). Tablets are typically blue, film‑coated, and round or diamond‑shaped, similar to sildenafil 100 mg formulations.
Onset of action: Onset is within 30 to 60 minutes after oral administration when taken on an empty stomach. A high‑fat meal can delay absorption and reduce peak plasma concentrations.
Duration of action: The therapeutic effect lasts for up to 4 hours, although erections lasting longer than 4 hours (priapism) are a medical emergency. The elimination half‑life is approximately 3 to 4 hours.
Alcohol recommendation: Alcohol consumption should be limited when taking sildenafil. Alcohol is a vasodilator and may increase the risk of orthostatic hypotension, dizziness, and headache, and can impair the ability to achieve and maintain an erection. Excessive drinking may reduce the efficacy of sildenafil.
Most common side effects: Headache, facial flushing, dyspepsia, nasal congestion, dizziness, and visual disturbances (colour tinge, increased sensitivity to light, blurred vision). These are usually mild to moderate and transient. Sildenafil is absolutely contraindicated in patients using any form of organic nitrates (e.g., nitroglycerin, isosorbide mononitrate/dinitrate) due to the risk of life‑threatening hypotension. It is also contraindicated in patients with severe hepatic impairment, recent stroke or myocardial infarction, or known hypersensitivity. Priapism (a painful erection lasting more than 4 hours) is a rare but serious adverse event requiring immediate medical intervention to prevent permanent tissue damage. Sudden vision loss in one or both eyes (non‑arteritic anterior ischemic optic neuropathy) has been reported rarely; patients with a history of this condition should avoid PDE5 inhibitors. Sildenafil is not approved for use in women in Canada, and no adequate studies in pregnant or breastfeeding women exist. The safety and efficacy of Femalegra for female sexual dysfunction have not been established by Health Canada.
