Generic Elimite ( Permethrin )
Elimite (permethrin 5% cream) is a topical scabicide indicated for the treatment of scabies infestations caused by Sarcoptes scabiei. Permethrin works by disrupting the sodium channel current in the nerve cell membranes of the mite, leading to paralysis and death of both adult mites and their eggs. It is effective after a single application in most patients. Elimite is available only by prescription in Canada and is intended for external use on intact skin only.
Usual adult dose: Apply a thin layer of Elimite cream to the entire body from the neck down, paying special attention to the webs of the fingers and toes, wrists, elbows, axillae, buttocks, and genital area. The cream should be left on for 8 to 14 hours (usually overnight), then washed off thoroughly with soap and water. One 30 g tube is generally sufficient to cover the body of an average adult. A second application may be considered after 7 days if live mites are observed or new lesions appear. All household contacts and close personal contacts should be treated simultaneously, regardless of symptoms, to prevent reinfestation. Bed linens and clothing should be washed in hot water and dried on high heat. Elimite should not be applied to the face, eyes, or mucous membranes.
Dosage form: Topical cream: 5% permethrin (50 mg per gram of cream). Available in a 30 g tube. The cream is white to off-white and odourless.
Onset of action: Permethrin kills scabies mites and their eggs within hours of application. However, pruritus (itching) may persist for up to 2 to 4 weeks after successful treatment due to the host immune reaction to dead mites and mite debris; this does not indicate treatment failure. New burrows or live mites after 7 days suggest possible reinfestation or treatment failure and require reassessment.
Duration of action: A single application left on for 8 to 14 hours is usually curative. Repeat application is recommended only if live mites are still present or new lesions develop after 1 week. The medication is washed off after the required contact time, and no residual effect is intended after removal.
Alcohol recommendation: No systemic interaction between topical permethrin and alcoholic beverages is known. However, patients should avoid applying alcohol-based lotions, astringents, or other irritants to the skin before or after applying Elimite, as these may increase local irritation, burning, or stinging at the treatment site.
Most common side effects: Mild and transient burning, stinging, itching, erythema, or tingling at the application site. These local reactions are generally due to the inflammatory response from the mite infestation itself and not to the drug; they typically resolve within 1 to 2 days after application. Rarely, contact dermatitis or a rash may develop in patients with sensitivity to permethrin, other pyrethroids, or any component of the cream base. Elimite should not be used on broken skin, open wounds, or areas of severe excoriation unless directed by a healthcare professional. It is not recommended for use in infants younger than 2 months of age. Accidental contact with the eyes should be avoided; if contact occurs, flush thoroughly with water. The cream is for external use only and should not be swallowed.
