Generic Buscopan ( Hyoscine butylbromide )
Buscopan (hyoscine butylbromide) is an antispasmodic agent indicated for the relief of spasms and cramps in the gastrointestinal tract, biliary tract, and genitourinary tract. It is used to manage symptoms of irritable bowel syndrome, biliary colic, renal colic, and bladder spasms. Hyoscine butylbromide works by blocking muscarinic receptors in smooth muscle, reducing acetylcholine-mediated contraction and thereby relieving painful muscle spasms. Because it is a quaternary ammonium derivative, it does not readily cross the blood-brain barrier and therefore causes fewer central nervous system effects than other anticholinergics such as atropine. Buscopan is available without a prescription in Canada for the temporary relief of abdominal cramps.
Usual adult dose: The recommended oral dose is 10 mg to 20 mg taken three to four times daily as needed for cramping. The tablets should be swallowed whole with water and may be taken with or without food. The maximum total daily dose is 80 mg. Treatment should be limited to short-term use; if symptoms persist for more than 2 weeks, the patient should consult a healthcare professional for further evaluation. Buscopan is not intended for continuous daily use without medical supervision. The 10 mg tablet allows flexible dosing for intermittent symptom relief. If a dose is missed, it should be taken as soon as remembered, but two doses should not be taken at the same time.
Dosage form: Film-coated tablets: 10 mg (white, round, biconvex, imprinted with "10" or "B"). Available in blister packs or bottles of 20 or 60 tablets. Buscopan is also available as a 10 mg suppository and as an injectable solution (20 mg/mL) for intramuscular or intravenous use in clinical settings, but the oral tablet is the most common over-the-counter form.
Onset of action: Symptomatic relief of gastrointestinal cramping typically begins within 30 to 60 minutes after oral administration. Peak plasma concentrations are reached approximately 2 hours after dosing. The onset is slower than intravenous administration but is adequate for most ambulatory patients with intermittent cramping.
Duration of action: The antispasmodic effect lasts approximately 4 to 6 hours, supporting dosing three to four times daily as needed. The elimination half-life of hyoscine butylbromide is approximately 5 hours, although the pharmacodynamic effect may be shorter depending on the target organ and individual response.
Alcohol recommendation: Alcohol should be used with caution during Buscopan therapy. Alcohol can irritate the gastrointestinal lining and may worsen underlying abdominal pain or cramping. It may also potentiate the mild sedative effects and dizziness that can occur with anticholinergic medications. Patients should limit alcohol intake, especially if they experience drowsiness or blurred vision.
Most common side effects: Dry mouth, blurred vision, constipation, dizziness, and tachycardia. These anticholinergic effects are generally mild and transient at recommended doses. Buscopan is contraindicated in patients with known hypersensitivity to hyoscine butylbromide, myasthenia gravis, untreated narrow-angle glaucoma, or mechanical bowel obstruction. It should be used with caution in patients with prostatic hyperplasia, urinary retention, or cardiac disease, particularly tachyarrhythmias. Because Buscopan can reduce sweating and impair thermoregulation, patients should avoid strenuous exercise and excessive heat during treatment. The tablets are not recommended for children under 6 years of age without medical advice. In rare cases, severe allergic reactions including anaphylaxis may occur; patients should seek immediate medical attention if they develop rash, swelling, or difficulty breathing. Buscopan is available without a prescription in Canada, but persistent or severe abdominal pain requires medical evaluation to rule out serious underlying conditions.
