Generic Beclate Inhaler ( Beclomethasone dipropionate )
Beclate Inhaler (beclomethasone dipropionate) is a synthetic corticosteroid with potent anti‑inflammatory activity indicated for the maintenance prophylactic treatment of persistent bronchial asthma in patients who require long‑term control. Beclomethasone dipropionate is a prodrug that is rapidly hydrolyzed to its active metabolite, beclomethasone‑17‑monopropionate, which binds to glucocorticoid receptors in the airways. This action inhibits the release of inflammatory mediators, suppresses eosinophil migration, and reduces airway hyperresponsiveness, edema, and mucus production. Regular daily use decreases the frequency and severity of asthma exacerbations. It is not a bronchodilator and should not be used for the relief of acute bronchospasm.
Usual adult dose: Dosage is individualized and titrated to the lowest effective dose that maintains asthma control. For the 200 mcg per actuation strength, a common starting dose is 200 mcg (1 inhalation) twice daily, or 400 mcg twice daily for more severe disease. The maximum recommended daily dose is 1000 mcg twice daily (i.e., up to 5 inhalations of 200 mcg twice daily). After clinical stability is achieved, the dose should be reduced to the minimum effective level, which may be as low as 200 mcg once daily in some patients. Patients must be instructed to rinse the mouth with water and spit it out after each use to reduce the risk of oropharyngeal candidiasis and dysphonia.
Dosage form: Pressurized metered‑dose aerosol inhaler delivering 200 mcg of beclomethasone dipropionate per actuation. Available in canisters providing 120 or 200 actuations. A dry powder inhaler formulation may also be available under other brand names.
Onset of action: Initial improvement in asthma symptoms and lung function may be observed within 1 to 2 days of starting therapy, but maximum therapeutic benefit is typically achieved after 1 to 2 weeks of continuous use. Patients should be counselled that the medication must be used regularly to prevent symptoms, even when they are feeling well.
Duration of action: The anti‑inflammatory effect is maintained throughout a 12‑ to 24‑hour dosing interval, supporting twice‑daily or, in some cases, once‑daily maintenance dosing. The clinical effect is determined by local receptor occupancy, and the plasma elimination half‑life of the active metabolite is approximately 2.8 hours.
Alcohol recommendation: There is no known direct interaction between inhaled beclomethasone dipropionate and alcohol. However, excessive alcohol intake may worsen gastroesophageal reflux disease (GERD) and can exacerbate underlying asthma symptoms. Moderation is recommended.
Most common side effects: Oropharyngeal candidiasis (thrush), dysphonia (hoarseness), throat irritation, and cough. These effects can be minimized by proper inhaler technique and rinsing the mouth after each use. Prolonged use of high doses may lead to systemic corticosteroid effects such as adrenal suppression, growth retardation in children and adolescents, decreased bone mineral density, cataracts, and glaucoma. The dose should therefore be kept at the lowest effective level. Patients transferring from oral corticosteroids require slow tapering of the systemic steroid to avoid acute adrenal insufficiency. Beclate Inhaler is not a rescue medication; a short‑acting bronchodilator should be available for acute symptoms. If paradoxical bronchospasm occurs immediately after inhalation, the medication should be discontinued and a healthcare professional consulted promptly.
