Generic Azelex ( Azelaic acid )
Azelex (azelaic acid) 20% cream is a topical agent indicated for the treatment of mild‑to‑moderate inflammatory acne vulgaris. Its mechanism of action is not fully established, but it is thought to work through several pathways: it possesses antibacterial activity against Cutibacterium acnes (formerly Propionibacterium acnes) and Staphylococcus epidermidis, normalizes the abnormal follicular keratinization process, and exerts an anti‑inflammatory effect by reducing the production of reactive oxygen species. These combined actions help to reduce the formation of microcomedones and resolve inflammatory acne lesions.
Usual adult dose: Apply a thin layer of Azelex 20% cream to the entire affected area twice daily, in the morning and evening, after the skin has been thoroughly cleansed and patted dry. A gentle massaging motion should be used to spread the cream evenly. A pea‑sized amount is typically sufficient for the entire face. Patients should be informed that a temporary sensation of warmth, tingling, or mild stinging may occur immediately after application, particularly on inflamed skin. Clinical improvement is usually observed after 4 to 8 weeks of continuous therapy; optimal results may require several months of use. If persistent irritation or lack of response occurs, a healthcare professional should be consulted. The hands should be washed after applying the cream unless the hands are being treated.
Dosage form: Topical cream: 20% concentration (each gram of cream contains 200 mg of azelaic acid). Available in tubes of 30 g and 50 g.
Onset of action: An initial reduction in inflammatory lesions and comedones may be seen within 4 weeks of consistent twice‑daily use. Maximum therapeutic benefit is typically achieved over 12 to 16 weeks of continuous treatment.
Duration of action: The therapeutic effect is maintained with regular twice‑daily application. After satisfactory control of acne is achieved, continued maintenance therapy may be required to sustain improvement.
Alcohol recommendation: There is no known systemic interaction between topical azelaic acid and the consumption of alcoholic beverages. However, patients should avoid the use of topical products containing high concentrations of alcohol, astringents, abrasives, or peeling agents on treated areas, as these can increase skin irritation, dryness, and discomfort.
Most common side effects: Transient local skin reactions such as mild burning, stinging, itching, erythema (redness), dryness, scaling, and peeling. These effects are most pronounced during the first 1 to 2 weeks of therapy and generally diminish with continued use. Azelaic acid can cause a reversible hypopigmentation (lightening of the skin), which may be more noticeable in patients with darker complexions. Patients should be advised to avoid occlusive dressings and to minimize exposure to environmental irritants. The cream should not be applied to broken, sunburned, or eczematous skin, and contact with the eyes, mouth, and mucous membranes must be avoided. If severe irritation or a contact allergic reaction develops, treatment should be discontinued and appropriate medical advice sought. Azelaic acid is not a primary irritant; however, some patients with sensitive skin may experience a burning sensation that can usually be managed with a reduced application frequency or the use of a gentle non‑comedogenic moisturizer applied at a different time.
